AT THE BEGINNING OF THE ACADEMIC YEAR:
• Student Information Form.
• Health Information Form.
• Certificate of physical fitness to perform physical education issued by a registered physician.
• Dental health Certificate.
• Authorisation to perform physical education Form.
• Authorisation to leave early on rainy days of Physical Education classes.

(*) must undergo legalization or be attached an Apostille.
(**) All documents issued in languages other than Spanish must be translated by a registered public translator, certified by the Translators' Association.
